  • Origin, date and script: Based on note in cursive script (f. 1r) "Anno xv c twie ende tsestich soe hebben die kerckmeesteren van Oistgheest als Sijmon Lourissoen wonende op poelgeester geest Adriaen maertenssoen wonende op die morsch ende Cornelis claessoen wonende an die vijnckewech dit bouck besteet om te scrijven heer adriaen adriaenssoen socius ende capellaen van Sinte marien poel buijten leyden gelegen inden voorseiden ambocht ende is voleijnt anno utsupra als pastoer was heer gerrit willemssoen geboren tot leyden ende coster was pieter nannessoen geboren tot Aerlanderveen. [signé] Adriaen maertens [?] zoon". - A second manuscript of the same scribe (there named Adrianus Goudanus), written in 1556 for Mariënpoel, is kept by the Academy of Sciences in Vilnius, ms. 22.95.
  • Based on the contemporary foliation ff. 1-5 are missing. - The lacuna between f. xvi and xxvi is a mistake in the foliation.
  • Last quire damaged by water.
